1. Please download the nominating form here (Requires Microsoft Word).  Included are some factors for your consideration.
   If you do not own Microsoft Word open and print this PDF form. Use the instructions here to complete form and mail to:

2. Please fill in the blanks provided by first clicking at the blank, including the second line for the home address, rather than use the “Enter key”.
3. Ignore the reference to K of C terminology such as “Brother” and “Council”.
4. Leave “Membership Number” blank.
5. Leave the month selection in the “Select One” text box blank.
6. Please limit the write-up (reasons) to the 11 lines given.  The text will properly scroll in this section.
7. Save the document and email it to jrenzi@verizon.net.
8. Random drawing of entries at the next Council business meeting will make the selection of the “Family of the Month”.  Entries not drawn will be saved to be included in the random drawing at the next monthly business meeting.
